You’ll be seen by our providers and establish a care plan for your blood pressure. When medically appropriate, you’ll be referred for blood pressure monitoring and receive a free device for as long as you are enrolled and participating.

Once you're enrolled, we’ll ship your personal blood pressure monitor straight to your door. A coach will then call to guide you through your first reading, ensuring you're comfortable and confident using it. The program is designed to be simple and easy to fit into your daily routine, making blood pressure monitoring effortless and stress-free.
